BASIC FUNCTION:This position is responsible for performing pharmacy program quality improvement activities including but not limited to delivering comprehensive Medication Therapy Management (MTM) services to physicians and members via outbound and inbound communications. This position is responsible for performing comprehensive medication reviews of individual member drug use and developing a Medication Action Plan (MAP) for consideration by the prescribing physician or primary care physician to decrease potential risks induced by medication use or gaps in care and optimize health outcomes.

JOB REQUIREMENTS:Registered Pharmacist in the state or eligible for licensure.Experience using window based word processing, spreadsheet and relational database software.Drug information skills.Strong Verbal and written communication skills.Knowledge of practice guidelines or practice parameters; pharmacoeconomics; quality improvement processes and techniques.Knowledge of medication use evaluation principles.*MTM program experience or completion of MTM/Pharmaceutical Care Training preferred
