Job Description

Job Summary:

Provides coordination of patient care throughout the healthcare continuum.

Scheduling experience is strongly preferred. LOCATION IS IN ELK GROVE, IL.

Responsibilities:

  • Provides coordination of communications with patients, families, physicians, and healthcare team members.

  • Coordinates patient scheduling with appropriate resources or clinical services and maintains required documentation.

  • Supports standard patient access initiatives across the healthcare continuum.

  • Provides support with navigating the healthcare system and assists patients with utilizing available community support services.

Qualifications

Education:

  • Bachelor's Level Degree

  • Bachelor’s degree preferred but not necessary with appropriate experience, attitude, presentation, demeanor and work ethic

Work Experience:

  • Strong customer service in a health care setting. Medical practice preferred.
